Our Exercise Referral Scheme is a 12 week physical activity programme that introduces individuals to the benefits of physical activity in order to help reduce risk factors for chronic disease and manage existing medical conditions. The programme is designed to encourage people to become and remain more physically active in a friendly, safe and supportive environment.  

Who is the scheme for?

It is suitable for those who would benefit from exercise to help for the rehabilitation, treatment or prevention of certain physical or mental health conditions.

Common reasons for referral:

  • High blood pressure
  • Diabetes
  • Circulatory/respiratory diseases
  • Depression/anxiety
  • Musculoskeletal problems
  • Cancer

How it works

Your GP/nurse or healthcare professional will complete a Referral For Lifestyle and Exercise form. This form will be given you and will include contact details about our GLL Leisure Centres.

To join the programme please call/email/visit your nearest centre to book your FREE initial appointment with an Healthwise coordinator.  

The Healthwise coordinator will complete an initial assessment and prescribe suitable physical activity that can be carried out over a 12 week period. You will be set up with an Exercise Referral membership which gives you unlimited access to the gym, swimming and group exercise classes. You may require supervision based on your referral reasons, which the Healthwise Coordinator will arrange with you to access.

The Healthwise Coordinator will support you through the 12 weeks and complete a 6 and 12 week assessment with you to check your progress. Once your 12 week programme is complete you can continue with your membership at a discounted rate to help you continue with your positive lifestyle change. 

How much does it cost?

Payment is available in three options:
• An annual membership of  £250
• A monthly Direct Debit for £25 per month for the first year
• A Pay & Play membership of £3.80 per visit

In addition to the main GP referral programme, our specialist programmes have been developed to meet the health needs of the local community. These programmes include:

  • Cardiac Phase IV Rehabilitation Classes
  • Falls Prevention Classes
