Welcome to the Healthier Together Hackney Scheme. If you have received the link to this page, then you have started, or are due to start your 12 week health intervention with us soon.

If you are enrolled on to a 12 week lifestyle weight management course, then you can expect regular contact with a dedicated instructor, who will present to you a different workshop every week for the next 12 weeks. This will help you learn about many different tools and techniques to help you develop habits and lifestyle choices which will improve your health. These sessions will also include physical activity in the gym or studios. Your first two weeks will be your induction weeks, where we will make sure you know how to use the gym fully, in order to make the most out of your healthy lifestyle approach.

Alternatively, you may be enrolled on to our physical activity scheme, where you will have a one to one appointment with an instructor, who will learn all about your needs in order to help you plan your own objectives and strategies. You will also be inducted into the gym, so that you are well versed with how to safely use the equipment, with a programme designed to support your physical and mental health needs.

You can expect two follow up calls over your 12 week experience, and you can also expect to see you instructor two more times after your initial assessment, to help measure your improvements, and to offer further support and sign posting.

In both cases, you are entitled to a discounted membership, to support your access to the gym, pools and fitness classes across all of our centres in Hackney. You will be eligible for this membership if you are new to the scheme, or have not received this already in the last two years.

How to get the most out of your experience

To get the most out of your experience we recommend the following steps:

  • Be punctual to your appointments. This will help you maximise your time with our expert instructors, who want to give you the maximum time possible to help you.
  • Avoid missing appointments. Your health is your priority, and your key to a long and comfortable life. So we recommend being kind to yourself, and putting your health first, by attending every session. If you end up missing appointments, then your membership will be cancelled, and we may have to discharge you back to your GP.
  • If you need to rearrange an appointment, we advise you to email us on hths@gll.org, or call us on 0203 903 7245.
  • Always wear or bring clothing and footwear suitable for exercise. Activity is very important for improving your health, even if it is gentle exercise. We will want to take you to the gym on every appointment in order to help you as much as we can
  • Attend one of our sessions on the Be Active time table. We have various classes and supported gym sessions which are available only to you.
  • Attend one of the mainstream sessions that you can also book on the app. The rest of our services in Hackney are available to you if you are enrolled on to the three month free membership.
  • If your health changes, please make sure you inform us. Your safety is important to us, and we prefer to work with your GP or health professional to ensure that you stay healthy. This will also help us to know when you need to pause the scheme, and we can restart you when you are fit, and able to participate.
  • Download the Better UK App before you come to your first appointment. This is the one stop shop for bookings, membership card and leisure centre access. It is very user friendly, but you will need a smart device to use it. Alternatively, access to your membership, and the booking function can be access via better.org.uk/login.
  • Follow your local leisure centre on Facebook. Type the name of your leisure centre into the Facebook search bar, and you will be able to find us, and see all the recent posts and initiatives that we share which could benefit your and your health. This is a good way to see helpful local innovations and initiatives in the community, as well as recruitment information, tips, testimonials and good news.

How we use your information

We collect information about you (your personal data) from all our Community Services. This includes information about your referral, assessments, diagnoses, activities and, in some cases, your answers to questionnaires. (for example, taking a blood pressure test).

We do this in order to provide safe care as well as assess the effectiveness of our care so that we can provide you with the best possible care and ensure that we can continually improve our services.

As this service is publically funded, and falls under the Community Services Data Set (CSDS). We, like many community health care providers, are obliged to share a certain amount of data with NHS digital, which is the central organisation that receives the same data from all publicly-funded Community Services across England. NHS Digital removes all identifying details and combines the data we send with the data sent by other care providers, forming the (CSDS).

The CSDS data set is used by NHS digital to produce anonymised/pseudonymised reports this means it is impossible to identify any individual patient in the reports. Such as reports that only show summary numbers of, for instance, service users referred to different types of services.  These reports help us, and the NHS, to improve the care we provide to you and other service users.

The benefits of the CSDS to you as a service user include:

  • making sure Community Services are available to everyone in all areas by measuring the care that is being delivered
  • better care, through monitoring progress to allow future services to be planned
  • informing patients about the care offered at different hospitals
  • more personalised and better organised care for people through understanding what care is needed nationally, for example understanding how many patients who are discharged from hospitals then need looking after at home

How do I get my membership card?

You can download the Better UK App, and log in with the information that we provide to you. Your membership card is revealed when you click the circle that says ‘membership card’. You can then use this to swipe in at any reception gate or turnstile in Hackney.

What if I don’t have a smart phone?

Speak to a concierge at any leisure centre, who can link you up a plastic card with the membership number that we provide to you.

Can I be referred again?

We can accept new referrals for the same scheme 2 years after you have completed your programme (if you are referred for exactly the same programme). However, if you complete the physical activity referral programme, you can be referred to the Weight Management programme as long as you meet criteria. However, once you receive a 3 month free membership, you cannot receive it again. Your discounted direct debit membership can be re-joined for up to 2 years after you complete your programme.

How do I get my discounted membership, and what happens after the first 3 months?

You will be enrolled by your instructor in your first appointment, so please bring you bank details (account number and sort code) and a card to pay your first month with. If you are eligible, you will get an enhanced discount for the first 3 months. After 3 months, you will automatically go up to the regular Healthy Living discount. In some cases, we might not be able sign up everyone at once in a group session. In that event, you may instead be called by our team to set you up on a membership, we will always refer to your membership number over the phone, which will be provided in session 1 or 2 by your instructor. This is to help you feel safe that we are calling about your membership and for no other reason.
